When you book a consultation on our website, you submit your full name, email address, phone number, gender, location, and the procedure our service you are interested in.

You may provide your full name and email address in order to receive our newsletter via email.

Payment Information

GOVERNING LAW AND JURISDICTION

This Privacy Policy shall be construed and governed under the laws of the United States and State of California (without regard to rules governing conflicts of laws provisions). You agree that venue for all actions, arising out of or relating in any way to your use of our Site, shall be in federal or state court of competent jurisdiction located in San Francisco, California, within one (1) year after the claim arises. Each party waives any objections based on forum non convenience and waives any objection to venue of any action instituted hereunder to the extent that an action is brought in the courts identified above. Further by using this site, you agree that in no way does such use establish a professional, medical care, or physician-patient relationship between you and our practice.

G. Social Media Plugins. This website may use social media plugins (e.g., the Facebook “Like” button, “Share to Twitter” button) to enable you to easily share information with others. When you visit our website, the operator of the social plugin may place a cookie on your computer, enabling that operator to recognize individuals who have previously visited our site.
