Fertility in your 30s does not drop off a cliff the day you turn 30. The change is gradual through the early part of the decade, then it accelerates as you move through the mid-to-late 30s. How Does Fertility Change in Your 30s?

You are born with all the eggs you will ever have. That number declines throughout your life, and so does egg quality. In your early 30s, that decline is relatively slow. After the mid-30s, the rate picks up.

Two of the largest published datasets on frozen egg outcomes put numbers to that shift. Doyle et al. (2016) analyzed 1,283 warmed eggs and calculated live birth efficiency per egg at 8.0% for individuals under 35, 7.3% for ages 35 to 37, 4.5% for ages 38 to 40, and 2.5% for ages 41 to 42. Cobo et al. (2016) analyzed 1,513 warmed eggs and found 6.6% for individuals under 35, 6.1% for ages 36 to 37, 5.6% for ages 38 to 39, and 1.8% for those over 40. 

These figures come from egg freezing research, not from studies of natural conception, so they do not map directly to monthly pregnancy probability. What they show clearly is the direction and pace of change across the decade. For context on natural conception, ACOG reports that healthy couples in their 20s and early 30s have approximately a 25 to 30 percent chance of conceiving in any given cycle. By age 40, that drops to less than 10 percent per cycle.

The Doyle dataset shows the highest per-egg efficiency in the under-30 group at 8.2%. Cobo’s youngest bracket, under 35, shows 6.6%. Both confirm that younger eggs produce stronger outcomes. The shift after 35 is real, but your individual fertility picture involves more than age alone.

What Affects Fertility Beyond Age?

Age is the most significant factor in egg quantity and quality, but it is not the only one. Several medical conditions can reduce fertility independently of how old you are.

Endometriosis (a condition where tissue similar to the uterine lining grows outside the uterus) affects an estimated 10% of people with ovaries, according to the American College of Obstetricians and Gynecologists, and can reduce egg quantity and interfere with implantation. Polycystic ovary syndrome (PCOS) can cause irregular or absent ovulation, which can make conception more difficult, though many individuals with PCOS conceive with or without fertility treatment.

The sperm-producing partner’s health matters too. Sperm quality can decline with age and is affected by heat exposure, smoking, alcohol use and certain medications.

Lifestyle factors carry weight as well. Maintaining a healthy weight, avoiding smoking and limiting alcohol are consistently associated with better reproductive outcomes.

Tracking your menstrual cycle can provide useful information about cycle regularity and possible ovulation patterns. Irregular cycles or significant changes in cycle length are worth discussing with a provider.
