How IVF Works at Open Fertility Astoria
At Open Fertility Astoria, consultations, treatment planning, and monitoring take place locally at our Astoria clinic with your dedicated Open Fertility provider. Egg retrieval, embryo transfer, and all embryology are supported through Spring Fertility’s Manhattan facilities. This gives you the convenience of neighborhood monitoring in Astoria with access to Spring Fertility’s Manhattan embryology program.
A typical IVF cycle involves eight steps.
01 Initial Consultation
Meet with your Open Fertility provider to complete initial testing and discuss what to expect. Your consultation takes approximately one hour. Semen analysis may be recommended as part of the evaluation for sperm-producing partners and will be discussed at your consultation.
02 Treatment Plan
The results from your initial visit — including your ovarian reserve, hormone levels, medical history, and reproductive goals — allow your provider to build a personalized treatment plan and calendar that reflects your clinical picture and your schedule.
03 Ovarian Stimulation
You will self-administer hormone injections for 10 to 14 days to stimulate egg production. Your care team teaches you how. Progress is monitored with transvaginal ultrasounds (internal ultrasounds providing a detailed view of the ovaries) and bloodwork throughout.
04 Egg Retrieval
Once your follicles have reached the appropriate stage of development, a board-certified Reproductive Endocrinologist performs your egg retrieval under general anesthesia at Spring Fertility’s Manhattan facilities near Bryant Park. The procedure takes approximately 20 minutes. Many patients are able to return to work or normal daily activities the following day, though recovery varies. After your retrieval, your care team will follow up with you about your recovery, results, and next steps.
05 Sperm Production
If a fresh sperm sample is part of your treatment plan, your care team will provide specific collection instructions. Donor or previously cryopreserved sperm is prepared according to the treatment plan.
06 Fertilization & Embryo Development
Embryologists (lab specialists who handle fertilization and embryo development) fertilize your eggs with sperm. Fertilization may occur through conventional insemination or ICSI (intracytoplasmic sperm injection) depending on your treatment plan. Those that successfully fertilize begin growing into embryos over the following days.
07 Preimplantation Genetic Testing (Optional)
If you elect to test your embryos for chromosomal abnormalities, embryos that reach the appropriate stage of development may be biopsied and cryopreserved while testing is completed. Your provider walks you through what the results mean and helps you plan the next step once they’re in.
08 Embryo Transfer
Your embryologist and Open provider select an embryo for transfer based on available clinical, genetic where applicable, and embryology information. The procedure typically causes little or minimal discomfort, takes place at Spring Fertility’s surgical center, and is completed by a board-certified Reproductive Endocrinologist while you are awake. A pregnancy test is taken approximately two weeks later.

What are the risks of IVF?

These may include ovarian hyperstimulation syndrome (OHSS), multiple pregnancy, ectopic pregnancy (a pregnancy that implants outside the uterus), and pregnancy loss. How long can my embryos stay frozen?
Embryo freezing has been performed since the 1980s. Births from embryos cryopreserved for over 10 years have been documented. Beyond 10 to 15 years, clinical experience is more limited, but existing data does not suggest detectable consequences from longer storage. Your care team can discuss storage timelines specific to your situation.
